Paul, the panel is wired incorrectly, the grounds and neutrals should be seperated and the Neutrals isolated from the panel, yes there would need to be a seperate ground (probably via rod(s)) serving this panel.

You do not need a seperate main disconnect at this panel.

How can you verify this? It looks like the feed is run through metal conduit (a conductive path).

I would tend to agree with Gerry that this is wired improperly unless you can verify the absence of the metallic path (which is well beyond the scope of an HI).

A GES is also required as you indicated.
