Defect:  It appears as if Romex has been installed and passes through what was once a sleeve for knob-and-tube wiring.  Therefore, I would want to see more of the wiring than what appears in this picture and would find every logical reason to have a licensed specialist evaluate the entire system.  I would ask myself if insulation could be covering additional wiring.  Recommend evaluation by a licensed, qualified specialist.
