I just got a non-commercial applicator (inspection for termites) license in Missouri. Is there a preprinted form to certifiy that a structure is free of termites or does one just state it in letter format. This was not covered in the course material supplied by the department of ag.

Thanks for the help. A local pest control mom and pop loaned me 25 forms until I get some (1-2 weeks).
