Hole for ventilation grate?
A Home Depot that I get into has one about that size on the ladies restroom door, with a return air in the ceiling of restroom. The air whistles through it all the time. Makes the door hard to open from the outside !!

Kinda makes the door not fire rated any more.

Good guess considering the above post.
I liked the doggy door. But there is a glass in it.

What happened? Had fun all day on the job with this one.

It's a door for small children to make sure the coast is clear when they're in trouble.
