Nick, How about thermal bridging?

Plastic under wood, as mentionned above is helpful, but remember that concrete takes years to cure and it is like a sponge,

If there is rain or high water in soil, water/humidity cold water out heat in hot water out cold in, condensation can happen.

We must remember that A foundation is reverse of a swimming pool.

Instead of keeping the water in to swim, the idea is to keep the water out to stay dry.
