I just looked at the pdf and the page numbers are wrong. The doc has 4 pages and the first three are numbered 1 of 3, 2 of 3, 3 of 3. The last page is numbered 4 of 4.

Assurance Check has created an automated form for use by NACHI members. It will be available at our future training course on Four Points.

Please send all of the information that is needed in section 11 of the form along with a digital copy of your signature to joe@assurancecheck.net and he will e-mail you back your own copy.
