I think it looks real. It looks like there is perspective to it and everything. The N is at greater distance and slant than the I at the end.

Plus it definitely looks like a blotchy shoulder (sorry Dee) which is characteristic of an unretouched photo.

The only quirk is the NACHI is more distinct and not blurry at all compared to the rest of the tat.
