Default piers

Found this in the crawl space. Not sure what to write. It doesn't look like the addition is bolted to the existing stucture and the pier has shifted. What would you write?

unless you're in a seismic area...
INFORMATIVE
"Structural framing of the addition did not appear to be bolted to the foundation as is required in modern building practice."

Was that girder laying down flat on its side or standing up on its edge?
Insulation has the wrong side down too.
