What is this? Next to a direct vent natural gas fireplace.

Appears to be a variable speed switch for the blower motor???? my guess

That was my best educated guess. Not sure what the black toggle slide switch is at the bottom, though.

How about a speed control so the fan can be slowed .

Are you certain it is related to the gas fireplace?

I have seen similar controls for ceiling fans. I saw one mounted such as yours, and also thought it was for the gas fireplace. Fortunately, the homeowner was present and he stated it was for a ceiling fan that had been removed.

I'm not certain about anything with it, hence the post. Just an assumption, because it was next to the fireplace.

looks like switched photoelectric sensor
test using electrical tape to block out the light and see what it controls, may have to "monkey" (proven inspector technique taught by Jim McKee) with the switch also

or antenna outlet
