Looks pretty typical for a panel of its age. People wrench on stuff over the years, and things go downhill from there.

The panel on the left was fed off the service line on the right. Tied in with split bolts. No wire clamp on left panel service wire & knock out was only 1/2 way out. The service wire was tight against the sharp edge of the knock out.
