Grounded and grounding conductors are required to be bonded (connected) at the service equipment, but this is definitely not the proper way to accomplish it.

Grounding conductor should terminate at ground bar, correct?

Is that solid copper conductor in a raceway? If so that would also be a problem since conductors larger than #10 in raceways are required to be stranded.

A simple question: In a 200 amp main panel, would there ever be an expectation for more than 100 amps in each of the balanced legs?

With a 200 amp panel at 240 volts you would have a capacity of 200 amps. At 120 volts you would have a capacity of 400 amps.
