Default GFCI height requirement

Is there a minimum allowable height for a free standing GFCI attached to a post on the exterior of the home? I had one at an inspection this morning that is 2 inches off of the ground. It doesn't appear to be correct and I believe that it should be a minimm of 12" off of the ground. I was trying to find guidance in the code book but couldn't locate anything.
Any help would greatly be appreciated.

Is the box and wire exposed? If so, is the box weathertight and what type of wire feeds the box. Here all NMD/NMW wire is required to be protected if less than 5 feet above ground/floor.
