How to report missing dead front knock out?

This is a case where the knock-outs in the dead front were removed some time ago and the lug screws were taped over with electrical tape. It’s a DLR situation. How would you report this or suggest a repair?

Though I can’t tell from the photo, I assume the tightening screws for the main lugs would be visible were it not for the tape. This being true, you already wrote your comment:

The knock-outs in the dead front (cover of the main panel) *were removed some time ago and the lug screws were taped over with electrical tape. *This is not a proper repair and is considered a major safety hazard as live components of the electrical system are potentially exposed. Recommend repairs by a qualified electrician to prevent injury/electrical shock.

This is correct. Thank you for your reply.

You might want to refer to them as twistouts to be more accurate Greg.

Does that term only apply to the dead front?

Can’t tell from pic if there’s room…

Purchase and install… simple!

If not enough room behind cover (and age of cover/panel) remove cover and have a new knock-out welded in place, as finding a replacement cover may not be easy. (Any muffler shop should do it for $5-$10 buckeroos).

Yes - the twistouts are fro breakers, the knockouts are for cable clamps.
