That must be one of those expansion fittings I keep hearing about

Looks like it never got properly seated in the adapter. If it was really glued in there something else would have broken.

When you see the pipe pulled apart like that, there is a bigger issue almost always afoot.

When the ground settles and pulls the conduit apart like that, is also tightly stretches the conductors. Meter bases have almost zero room for slack, so the conductors will be stretched up like a banjo string. They can and do bend the lugs in the meter base toward the center...toward the grounded neutral. About 25% of the time the correction to this problem not only involves installing a proper expansion fitting but it often involves replacing the meter can or the meter can interior components due to the stresses places on them by the tight conductors.
