Default insulating a slab with radiant heat

What is the proper method for insulating a concrete slab home with radiant floor heat?

yesterdays inspection has an exposed area of the exterior slab, insulation cut short... and it is melting the snow away from the home... heat loss?

Is that 1" of foam insulation showing above grade? I hope that its not the slab protruding out from under the siding.
