Anyone have a WDI/WDO agreement they'd be willing to share?

I don't know what's required in PA, but in Ohio we have to use the NPMA-33 form. I've used page 2 as the agreement.

Looks like Ben is now selling the WDI inspection agreement that used to be free here in the list of documents in the Inspector Mall section of the site. Contact me off the board and I will send you a copy of the one that I use.
