Default 5 gallon bucket used for floor drain cover

Question about an attached picture.

In a 1998 home in basement. A 5 gallon bucket lid used to cover access to shower drain. Poorly sealed with duct tape and water noted.

Water was visible through hole and was about 3/4 inches from top.

What verbiage would you use in your report?

I know they make hard covers for this. Was concerned if water is under floor or just in bucket or drain.

You already said it, "A 5 gallon bucket lid used to cover access to shower drain". If you include the picture, they should be able to figure it out.

Did a newer home with a proper cover at floor. Have pic for future reports.
