Concrete foundation wall- discoloration and blow-out

New home...
I thought this was just staining from run-off, but there's a lot of it, what is it, it looks like it might be related to the blow-out which exposed these bolts, which at first I thought was just caused by careless straightening of anchor bolts before concrete had a chance to cure completely. Between the two bolts a section of concrete deterioration is visible which looks like spalling or flaking. Chemical of some sort?

The area with the damaged bolts is located further down this same wall.
