Telling the client the outlet has reversed polarity is generally enough. Hot and neutral reversed also works well.

Paul, I wouldn't be so tough on this question, when I asked a very similar one 18 months ago less than 60% got it right, the explianation of the correct answer will help many people (members and non-members) understand the principles better.

Reversed Polarity --- I hate this way of talking about AC which changes polarity by it self. That is why it is AC

How about "Not wired properly"
