The form is great. however, you have to be careful because each jurisdiction may have their own tailor made regulations. Florida is based off the national ADA code. I would customize the form to each jurisdiction. Otherwise is a great start.

As for inspecting hotels, a section would be needed on the percentage of rooms that are accesible per ADA requirements.
