waiver for Thermal Imaging

Hey Just wanted to know.
1. Does anyone have the customer sign a waiver if they don't want thermal Camera used on a home inspection.
2. Do you offer the thermal camera as a additional cost or incorporate it into your inspection for more money?
thanks for any input on these questions. i am trying to decide how to market my thermal camera
