Here is a copy of our four point field form. We use this to take notes in the field while doing four points.

Great idea. Can save a lot of paper and printer ink.
And I like the addition of the "Brand".
Do you email the 5 + pictures page InterNACHI form to the agnets and/or client?
If so did you add your letterhead to the form?

My logo and name are on all of my forms. I do not add the last pages unless needed. I do send pictures with all of my reports.

Do you have the offfice form in a fillable pdf or a Word doc then convert to pdf to email?
