Likewise, for perfectly safe old equipment that is not up to current codes.

Totally agree. Safety must be our first priority. It is what we can get sued for.

And don't discount that the utility of the AHJ didn't do it. Plenty passes them right by. I see guys form our local utility doing stuff that would get a regular electrician fired. They are a monopoly, and they know it.
