I suspect the screw is to flush the breakers, but is there an issue?
The screw is to secure the back fed breakers as required.
That is just one style of breaker retainer that is required on a backfed breaker.
I am an electrician and this is acceptable for this type of panel. This holds the main breaker tight to the backrail.
