Today I inspected a home with an upgraded 200 amp panel. In another location was the original 100 amp panel which is now used as a sub-panel but no isolated ground and neutrals and missing fourth cable. Would this be a sub-panel mistake or what?
If it no longer the service equipment, it must be rewired as load side equipment.

This is what might be, although I can’t see where everything goes nor tell if the grounding and grounded conductors are separated (doen’t appear so) and grounded conductor isolated as needed. When in doubt refer it out but there appears to be 4 conductors:
