Looks like the lamp cord you guys use is missing a bushing at the bottom of that sub as well.

Good eye. That's at the bottom of the list for this one. Below double tapped feeders at main lugs, double tapped aluminum/copper at ground bar, 2-wire feed with ground at sub, neutral/grounds bonded at sub, double tapped breakers at sub, etc. etc.
