Does anyone have a template based on NPMA-33 (wood destroying insect) report? If so, would you be willing to share it?

Not sure about Indiana but in Ohio the WDO inspections have to be on the NPMA-33 form and can not be customized.
Very sorry I’m so late posting to this but I just now noticed it,
I’m in Indiana also, and the training I received from the Office of the Indiana State Chemist at Purdue University expressly mandated that the NPMA-33 form be paid for and used exclusively.
