Just did a 4-point for a customer, later found out the agent sent to Citizen’s… They of course didn’t accept the standard 4-point, but want it on THEIR form. I sent what I have, he responded that I should use the most current form. I use Homegauge, my form says Revised in 2012… does anyone know if there is a more current version? I asked the underwriter, and he won’t respond! Thanks

The form was updated almost a year ago. Try Citizens website.

Remember that is just a SAMPLE and you can use any form layout as long as it has the required info from their sample form. Check out the Florida Inspectors facebook page for a bunch of great downloads.

You may want to check a some of the Florida Inspector pages for a fillable version

I personally will not do them for that either BUT I am training a guy that will do them at a competitive price:D He wants the work and the experience.
